A fracture simply means a broken bone. Many people believe a fracture is only a small crack or partial break, but medically speaking, a fracture and a broken bone mean the same thing.
Fractures can occur in several forms. They may be displaced, where the bone pieces move out of position, or non-displaced, where the bone remains aligned. They can also be open fractures, where the bone breaks through the skin, or closed fractures, where the skin remains intact.
One of the most commonly missed types of fractures in the foot and ankle is a stress fracture.
A stress fracture develops from repetitive stress on the bone, rather than from a single traumatic injury. This often occurs in people who have recently increased their activity level, running, walking, or sports participation.
CAUSES
Understanding Stress Fractures
A helpful analogy is a wooden baseball bat. When a baseball player hits a ball with a wooden bat, the bat may develop a crack inside the wood. The bat might look normal on the outside, but when the player taps it on home plate, they can hear or feel that the bat is no longer solid.
A stress fracture occurs in a similar way. The bone develops tiny cracks from repeated stress, even though the break may not immediately be visible on imaging.
SYMPTOMS
Common Stress Fracture Symptoms
Stress fractures usually cause gradual pain that worsens with activity.
Patients commonly notice:
Pain during walking, running, or sports activity
Tenderness when pressing on the bone
Swelling in the foot or ankle
Pain that improves with rest but returns with activity
Increasing discomfort over time
Because the symptoms can start gradually, stress fractures are sometimes mistaken for tendinitis, sprains, or general overuse injuries.
DIAGNOSIS
Diagnosing Stress Fractures
Diagnosis for stress fractures often includes a detailed medical history and physical examination.
Evaluation may include:
A physical exam focusing on areas of tenderness (tuning fork)
Weight-bearing X-rays
MRI imaging if a stress fracture is suspected
One challenge with stress fractures is that X-rays may not show the fracture during the first 2 weeks. MRI imaging is often more sensitive and can detect stress fractures earlier.
Providing a complete history of your symptoms and activity level is extremely important in helping me make the correct diagnosis.
TREATMENT
Stress Fractures
Conservative Treatment
Most stress fractures of the foot and ankle heal well with conservative treatment. Treatment focuses on protecting the bone while it heals.
Common treatments include:
Activity modification or temporary rest
Immobilization with a walking boot or cast
Reducing weight-bearing on the affected foot
Elevation to reduce swelling
Gradual return to activity once healing occurs
Stability of the bone is critical because the fracture fragments need to remain aligned in order to heal properly.
Is Surgery Needed?
Most stress fractures heal without surgery.
However, surgical treatment may be recommended if:
The fracture is displaced
The bone does not heal properly
The fracture occurs in an area with poor blood supply
The injury is severe or unstable
Surgery may involve stabilizing the bone with screws, plates, or other fixation devices.
Healing Time for Foot + Ankle Fractures
Broken bones typically take at least four weeks to begin healing, but fractures in the foot and ankle often take longer. This is because these bones must support body weight during walking, and gravity can increase swelling in the lower extremities.
Keeping the foot elevated above the level of the heart when possible and following immobilization instructions carefully can help promote healing. With proper care and protection, most stress fractures heal successfully and patients can gradually return to normal activity.
