Tarsal Tunnel Syndrome
A Debilitating Cause of Medial Ankle Pain
Tarsal tunnel syndrome is a painful nerve condition that affects the inside (medial side) of the ankle.
It is often unilateral and can become debilitating. Many patients describe pain that begins after standing for a short period of time and worsens with activity. If left untreated, it can significantly limit walking, exercise, and daily function.
CAUSES
What is Tarsal Tunnel Syndrome?
Tarsal tunnel syndrome occurs when the tibial nerve becomes compressed as it passes from the leg into the foot through a narrow space on the inside of the ankle called the tarsal tunnel. Because this space is tight, any added pressure can irritate the nerve.
Common causes include:
Varicose veins in the tunnel
Flatfoot deformity
Swelling or inflammation
Scar tissue
Space-occupying lesions

SYMPTOMS
Tarsal Tunnel Symptoms
Patients often report:
Burning pain along the inside of the ankle
Tingling or numbness in the bottom of the foot
Pain after standing for short periods
Worsening symptoms with walking or exercise
Night pain
Unlike simple inflammation, this pain has a nerve quality that’s described as sharp, electric, or radiating.
DIAGNOSIS
Diagnosing Tarsal Tunnel Sydrome
Diagnosis requires a detailed foot and ankle examination, including specific clinical tests to identify tibial nerve irritation and determine the source of compression. Imaging and nerve studies may be used when appropriate.
Accurate diagnosis is critical. Chronic nerve compression will not improve with rest alone.

Conservative Treatment
Many patients improve with conservative treatment, including:
Compression socks (especially when varicose veins are involved)
Calf stretching
Activity modification
However, when conservative treatment fails and nerve symptoms persist, surgical decompression may be necessary.

Surgical efficacy analysis of tarsal tunnel syndrome: a retrospective study of 107 patients
This retrospective study of 107 patients undergoing tarsal tunnel decompression surgery reported that about 71% achieved good or very good symptom relief after the procedure. The main takeaway is that surgery is generally effective for many patients, but a significant portion still experience incomplete relief or variable outcomes depending on diagnostic accuracy and patient selection.
This review found that conservative treatments such as orthotics, NSAIDs, physical therapy, and corticosteroid injections are typically used as first-line management for tarsal tunnel syndrome before considering surgery. The key takeaway is that non-surgical care is standard initial therapy, but the supporting evidence is moderate and many patients ultimately require surgical intervention if symptoms persist.
